The three main suggested outcomes are:\n\nOutcome 1: Improved professional capacity of midwives to provide comprehensive and sustainable SRHR services including quality routine and emergency maternity care services.\n\nOutcome 2: Improved Basic Emergency Obstetric and Newborn Care (BEmONC) and Comprehensive eEmergency Obstetric and Neonatal Care (CEmONC) services available in Sida target districts through professional midwives and interdisciplinary teams.\n\nOutcome 3: Improved availability of high-quality, sustainable, human rights-based comprehensive and integrated SRHR information and services provided through professional midwives.\n\nThe project is implemented in the five most vulnerable districts in the country, covering 31 upzilas(sub-districts)These districts are adversely affected by climate-change, natural hazards, remoteness and inaccessibility due to poor infrastructure. \n\nThe project aims to build sustainable structures and capacity at the national and district-levels to introduce and scale up midwifery-led SRHR services, with climate-change adaptation and resilience as important components of the midwifery-led care (MLC) model. Particularly, the project strives to demonstrate the effectiveness of operationalizing government health facilities closest to the community i.e., Union Health and Family Welfare Centres (UHFWC), with the existing physical infrastructure for providing maternity care and other SRHR services through midwives. The project will not only act as catalysts in scaling up MLC in the public health facilities at the primary health care (PHC) level but also will act as a pull-factor for the midwifery profession by creating employment for them and in the process, contributing to womens empowerment.\n\nIn addition to above, UNFPA is working towards the following objective: to improve availability, accessibility and utilization of SRHR services and climate change resilience and adaptability for the young and marginalized people of Bangladesh through engagement between youth, authorities and other key stakeholders at every level of governance. This objective was added following the youth-led movement in July-August 2024, which sparked significant socio-political changes and led to the establishment of a more youth-focused interim government. Results contributing to this outcome remains to be seen.\n\nOutcome 1: Improved professional capacity of midwives to provide comprehensive and sustainable SRHR services \n\nProgress has been made in enhancing the professional competence, equity, and institutional integration of midwives during the reporting period. Below, some examples are listed:\n\nMidwifery Education and Institutional Capacity\n\nBetween October 2024 and March 2025, 550 midwife students received training on the impact of climate change on sexual and reproductive health and rights (SRHR), an increasingly relevant component of midwifery curricula given Bangladeshs climate vulnerability. \n\nThe previously developed Plan-Do-Study-Act (PDSA) quality assurance tool was introduced across 10 midwifery education institutions, each developing action plans for curriculum and service improvement. Five institutions achieved accreditation, and five additional schools began preparations for accreditation, strengthening long-term institutional quality and accountability.\n\nEquity and Inclusion in the Profession\n\nBetween October 2024 and March 2025, scholarships were awarded to 7 girls from ethnic minorities in Bandarban, allowing them to enroll in midwifery education. UNFPA's efforts also supported the Bangladesh Nursing and Midwifery Council (BNMC), resulting in enrolment of 25 indigenous students in 2024 from 8 districts, expanding access to professional midwifery education.\n\nLeadership and Professional Standards\n\nBetween October 2024 and March 2025, 25 midwifery faculty and managers received leadership and management training, fostering a new generation of midwife leaders. 25 officials from the Directorate General of Nursing and Midwifery (DGNM) participated in a three-day workshop on the prevention of sexual abuse, exploitation, and harassment (PSEAH), supporting rights-based workplace practices.\n\nThese advances build on long-term investments by Sida and academic partnerships with Swedish institutions, such as Dalarna University and the University of Gothenburg, which have helped embed international standards into local institutions since the mid-2000s.\n\nOutcome 2: Improved BEmONC and CEmONC services available in target districts through professional midwives \n\nBetween October 2024 and March 2025, emergency and routine maternity care services saw measurable improvements in coverage and quality in Sida-supported districts. Below, some examples are listed:\n\nShift Toward Safe, Midwife-Led Deliveries - SRHR services showing improvements compared to previous reporting period\n\nSignificantly, the caesarean section rate decreased by 13% during the project period, and there was an increase of 17% in the stabilization of eclampsia cases, suggesting improved clinical decision-making and access to routine care. Normal vaginal deliveries (NVDs) increased by 5%. Post-abortion care coverage and post partum family planning increased by around 25% respectively.\n\nMidwives as Primary Care Providers\n\nBetween October 2024 and March 2025, midwives handled the vast majority of childbirths in project districts, including:\n\n- 99% of NVDs att community level health facilities (Union Health and Family Welfare Centers). This represents a 19 % increase compared to the baseline before project inception\n\n- 78 % at primary health care level (Upzilla Health Complexes). This represents a 14 % increase compared to baseline\n\n- 49% at secondary level health facilities (Mother and Child Welfare Centers), and 85% at tertiary hospitals. This represents a 30 % decrease compared to baseline. The decrease is a positive effect of the project, indicating that uncomplicated deliveries are taking place at lower level facilities to a greater extent. \n\nIn partnership with the Directorate General of Family Planning (DGFP), UNFPA supported laboratories at Chandpur and Noakhali Maternal and Child Welfare Centres by supplying essential equipment and reagents. From October to December 2024, 3,466 tests were conducted, helping detect high-risk pregnancies and manage complications. In January 2025, the Government of Bangladesh took over operations, marking a successful transition to sustainable, government-led healthcare services.\n\nTo strengthen SRHR services in urban areas, UNFPA assessed three facilities under Dhaka South City Corporation. Recommendations included deploying midwives, upgrading infrastructure, and equipping facilities for round-the-clock care. As a result, Dhaka Mohanagar General Hospital was selected to pilot 24\u002F7 midwife-led NVD and SRHR services from Q2 2025. Four midwives were deployed, with the initiative expected to serve as a scalable, cost-effective model to improve reproductive health outcomes for underserved urban communities.\n\nMoreover, several promising developments occurred during the reporting period. Notably, the Directorate General of Health Services (DGHS) decided to:\n\n- introduce the Robson classification system for monitoring caesarean section rates in district hospitals,\n\n- adopt a new National Referral Guideline for Maternal Health\n\n- recruit four district SRHR officers to support professional development for midwives, with a particular focus on enhancing the skills and leadership of midwives.\n\nOutcome 3: Improved availability of high-quality, comprehensive SRHR information and services provided through professional midwives\n\nProgress towards the third outcome is progressing as planned. UNFPA has already surpassed its 2025 target for increasing the number of public health facilities meeting readiness criteria to deliver postpartum family planning (PPFP), post-abortion care (PAC), and menstrual regulation (MR) services. In project districts, only 24 Union Health and Family Welfare Centres have yet to meet these standards. Additionally, at all Upazila Health Complexes, midwives routinely screen pregnant women for HIV, syphilis, and hepatitis B.\n\nHowever, one area where progress is halting is UNFPA's work to prevent, screen and referr GBV. Only 26 % of facilities are doing this now (target for 2025 is 65 %). This is entirely linked to low number of GBV survivors seeking health care services. Mitigation measures has been introduced and is presented in the next section.\n\nIn climate-vulnerable Sunamganj, SRHR and GBV services were incorporated into local disaster plans. UNFPA also helped translate national MISP guidelines into Bengali for wider use in emergencies, strengthening a resilient, rights-based health system. In remote Bandarban, 113 deliveries were supported through Midway Homes, with 29 women receiving transport assistance, highlighting midwives crucial role in underserved areas.\n\nIn November 2024, following UNFPA advocacy, DGNM proposed creating 2,520 new midwife posts at UHCs and sub-centres to provide 24\u002F7 NVD and SRHR services.\n\nEfforts to adapt to and mitigate climate change by health systems strengthening is progressing. However, as this is still a new initiative, results are mostly at output level still. Including the training of 5,200 community members, 1,350 households and 900 adolescent girls and boys in 16 schools on SRHR impact of climate change.\n\nIn addition, 5 health facilities prepared climate action and emergency preparedness plans. Integrating SRHR into district and health facility climate and disaster plans offers a scalable, cost-efficient approach to enhance health system resilience and community preparedness to climate change and disasters. Also, International best practice of Minimum Integrated Sexual Package (MISP) for climate and humanitarian situations was introduced in Sunamganj. \n\nHighlights from the midterm evaluation (20172024)\n\nDuring the reporting time, a mid-term review was conducted. This evaluation highlighted the broad and significant impact of the project since its inception in 2017. It contained several chapters briefly captured below. \n\nA quantitative analysis of sexual and reproductive health and rights (SRHR) service utilization and midwife deployment trends between 2016 and 2024, using data from the Directorate General of Health Services (DGHS) and the Directorate General of Family Planning (DGFP), showing:\n\n- Midwives significantly boosted maternal health services in project districts between 2019 and 2024. ANC1 visits increased by 56% at Union Health and Family Welfare Centres (UHFWCs) and 76% at Upazila Health Complexes (UHCs).
