These dialogues led to real-time commitments to address issues such as absenteeism, facility readiness, and service gaps.\n\n- Systematic monitoring and follow-up: RC members conducted annual facility performance reviews, identifying specific quality gaps and following up with responsible authorities. The process has institutionalized a cycle of community-based monitoring and corrective action in health service delivery.\n\n- Strengthened community-level platforms: A total of 37 coordination meetings were held at Community Clinics and Union Health and Family Welfare Centres (UHFWCs), effectively activating previously dormant management committees. These meetings resulted in localized problem-solving and community ownership of facility-level issues.\n\n- Youth engagement and leadership: Youth Forums increased their visibility and effectiveness by participating in monitoring visits, conducting awareness campaigns, and supporting vulnerable populations. Their proactive roles in disaster response (e.g., flood relief, free medical camps) and blood donation drives strengthened their credibility and embedded them as long-term stakeholders in local accountability mechanisms.\n\n- Emergence of autonomous civic actions: RCs in districts such as Khagrachhari, Netrakona, and Manikganj demonstrated initiative beyond project mandatesadvocating for clinic relocations, equipping facilities, and supporting marginalized groups. This evolution from activity-based engagement to autonomous advocacy is a strong indicator of sustainable local accountability.\n\nExamples from Sunamganj\n\nIn Sunamganj, the district Health Rights Forum (HRF) introduced a citizens charter that provides clear information about the services available at the local clinic, such as malaria and dengue testing. This intervention has had a noticeable impact: the number of patients visiting the Sripur clinic has doubled, increasing from around 250 to approximately 500 patients per month. Alongside this growth, the quality of services at the clinic has also improved. Regular meetings between the community group committee and clinic staff have encouraged the doctor to increase his presence at the facility, further strengthening care provision. \n\nBuilding on previous efforts, the district HRF has driven several significant improvements in the area. These include electrifying the Sripur Clinic and restoring a dilapidated road connecting it to the Lakhmansri Family Welfare Centre. Similar achievements have been recorded in other regions, such as securing water connections, installing tube wells, and providing medical equipment to health care facilities.\n\nThe HRF has also actively collaborated with local residents to resist corrupt practices that hinder access to health care. These include challenges posed by private ambulance syndicates and widespread doctor absenteeism. Through ongoing dialogue with both doctors and ambulance providers, patients have reported fewer cases of doctors prioritizing their private practices over public duties, and exploitative behaviors by private ambulances are increasingly being challenged.\n\nObjective 2: Strengthened Media Engagement and Capacity to Promote Social Accountability in Health\n\nThe project has enhanced both the capacity and reach of media actors in health accountability discourse.\n\nIncreased quantity and quality of health journalism: Following targeted training workshops held in October and November 2024, local journalists from multiple RCs produced and published investigative reports highlighting systemic issues such as absenteeism, drug pricing, and local service failures. The publication of these reports in mainstream and regional outlets has broadened public understanding and pressure for reform.\n\nStronger media-health sector interface: A national-level workshop on investigative health journalism, held in July 2024, equipped 27 journalists with thematic expertise and advanced reporting skills. This capacity-building resulted in direct media outputs on topics like antimicrobial resistance and informal payment practicesissues previously under-reported.\n\nStrategic alliances with national media actors: BHWs collaboration with national-level outlets led to increased visibility of underrepresented health issues. This partnership has established a media feedback loop that aligns journalistic outputs with citizen grievances and policy advocacy.\n\nInstitutionalized journalist engagement: The creation and activation of Health Reporters Forums and consistent involvement of journalists in public hearings and policy dialogues have institutionalized media as an integral actor in health accountability efforts.\n\nObjective 3: Strengthened Advocacy Work to Accelerate the Implementation of UHC by the Government\n\nThe project achieved significant milestones in evidence-based and legal advocacy, influencing both policy discussions and institutional reforms.\n\nLocal health budget tracking for policy influence: A study analyzing local-level health budget allocations in Kurigram district has been completed and is now being disseminated. Findings from the study are expected to serve as a foundation for evidence-driven advocacy on increasing health care spending, notably on primary health care.\n\nStructured dialogues on absenteeism and retention: Through eight district-level workshops with health managers and service providers, actionable recommendations were developed to address absenteeism and workforce retention. These regional dialogues have created a channel for ground-level insights to inform national policy processes. BHW are now preparing for a national policy dialogue to present regional findings. A draft action plan on absenteeism will be shared with the development partner anti-corruption coalition, headed by Sweden, ensuring coordinated advocacy efforts.\n\nNational policy momentum through legal mechanisms: The projects engagement in Public Interest Litigations (PILs) has resulted in tangible institutional commitments, particularly in road traffic emergency care and C-section regulationissues central to Universal Health Coverage (UHC) equity and access goals. However, these commitments are yet to be translated into action. \n\n- Caesarean Section (C-section) guidelines endorsed by the High Court were translated into Bangla, facilitating public access and understanding. The process signifies the successful completion of a legal advocacy cyclefrom petition to policy change to public dissemination.\n\n- Emergency Services Protocol for Road Crash Victims was developed, endorsed by the Ministry of Health, and is now pending piloting.
