The final goal of the outcome is to pilot the transfer of build-up services to the local de facto structures, ensuring their sustainability, but also shaping an augmented engagement between CSOs and LPSs. \n\nThe cross-river cooperation between the CSOs from both banks will be further supported via sharing and learning (joint grants, study visits, transfer of knowledge, mentoring and coaching). The Platform for Sustainable Community Development (of 39 CSOs and initiative groups from both banks of the Nistru river working with various vulnerable groups) will be supported via its Secretariat and thematic groups to advocate for social changes. \n\nOutput 2. This added a new dimension to the Programme's activities, primarily carried out online,which adversely affected interaction with partners and beneficiaries. However, when situation permitted, face-to-face activities were organized, fostering closer relations and cooperation between CSO leaders and human rights activists from both banks. It has to be mentioned that the security concerns forced some of the staff of the Programme to be more reluctant to travel to the region, while some of the local CSOs partners reduced their activity or left the region. Also, some delays in the implementation of the One UN JA activities were caused by the newly introduced internal procurement system. The interaction with the Sustainable (Community) Development Platform (SCDP) was reactivated, expanding its membership and outlining future activities. An important change was the reduction of Thematic Working Groups number from three to two due to overlapping membership and complementary activities of the Adjustment of the Normative Framework and Advocacy groups. The awareness of the members of the Sustainable (Community) Development Platforms members was raised in the area of human-rights-compliant community-based services, by organising a roundtable in Tiraspol on Human Rights and Social Services with the participants from both banks of the Nistru River. From mid-2023, the trust line for the victims of gender-based violence in the Transnistrian region was supported, receiving 306 domestic violence-related calls and managing 18 SOS cases for women. In the same area guidelines for sexual violence support were developed and presented during a workshop. IOM also provided burnout prevention training to NGO staff in December due to increased workload from the Ukraine war, improving their ability to assist those in need and collaborate effectively. Work with CSOs, working or representing the PLHIV continued in the Phase II of the One UN JA. Key activities included organizing training sessions for their staff and beneficiaries to deepen the understanding of CLM principles and identifying critical areas for improvement in HIVrelated services and human rights. A targeted questionnaire was developed to assess awareness among PLHIV, revealing ongoing challenges in disclosure, employment, and testing procedures.\n\nThe Programme actively supported human rights defenders and activists in the Transnistrian region through various initiatives. Key activities included capacity-building for disability rights, creating a cross-river referral pathway for refugees with disabilities, and strengthening partnerships to aid Roma communities in obtaining identification documents. To improve service delivery of CSOs, the Programme supported the institutionalization of the Roma community mediators network in the Transnistrian region. This included consultations, renewing support through the Roma CSO Shatior over Nistru, and facilitating access to various services for both local Roma and Roma refugees from Ukraine. Cross-river cooperation with mediators from the right bank of the Nistru River was strengthened, and the network received positive feedback at a national forum. Additionally, Shatior over Nistru underwent a PSEA assessment to enhance its capacity and align with UN standards, with ongoing support planned for the next year. The three Crisis Rooms were established to provide temporary shelter and support for women who use drugs (WUD) and their children, facing domestic violence. These rooms also accommodated refugee women and children from Ukraine. Consultations were carried over to transfer this service to local administration. To ensure the continuation of the efforts initialised in the first phase of the Programme, in close cooperation with the Ministry of Justice an Informative Note on community reintegration for people released from detention in Moldova was finalised and disseminated. This document outlines six key interventions: family support, housing and financial assistance, health and social services access, substance abuse and behaviour change programmes, job placement assistance, and monitoring of ex-prisoners. A new element was introduced in the Phase II of the One UN JA - work with young people on sexual and reproductive health. An out-of-school comprehensive sexuality education (CSE) programme for 60 young people from the Transnistrian region was developed and implemented. This initiative addresses misinformation and promotes informed decisions about sexual and reproductive health and involved young people from various vulnerable backgrounds. Feedback indicates increased awareness and confidence among participants. Additionally, a Facilitators Manual was translated into Russian to support the programme. Additionally, 10 CSOs working with young people have been empowered and increased their capacity to integrate CSE in their programmatic activity and promote healthier behaviours and relationships. To enhance CSOs advocacy actions, the Programme facilitated the development of a Human Rights Framework (HRF) with the participation of local CSOs and human rights defenders. The draft HRF, consulted with de facto structures, is under consideration and will be finalized in early 2024. Concurrently, CSOs from both sides of the Nistru River contributed to creating a new human rights framework for Moldova, addressing specific issues in the Transnistrian region. Additionally, new human rights education curricula were introduced in collaboration with the State University of Medicine and Pharmacy to combat stigma and discrimination among future medical professionals. Four training sessions were held for doctors and nurses in Tiraspol and Ungheni, enhancing their communication skills and promoting non-discriminatory practices, as \nhighlighted by the Stigma Index 2022. This initiative underscored the commitment to improving medical services and respecting patient rights, particularly for those affected by HIV. A significant step in bridging healthcare initiatives and fostering inclusivity in the region has  been successfully implemented through a collaborative initiative aimed at empowering 149 doctors and nurses from both banks of the Nistru River with the essential skills and knowledge to cultivate a supportive and non-discriminatory environment for individuals affected by HIV. The Roadmap for Child Rights in the Transnistrian region, developed during the previous phase of the One UN JA and validated by de facto structures, faces challenges in its approval and implementation. Despite participatory development and validation by the de facto structures,  the Programme still face challenges. The implementation of the Roadmap requires significant \namendments to the de facto normative framework, shifting towards gender-sensitive and childfriendly justice, which will impact front-line workers and justice sector professionals. Additional advocacy efforts, including roundtable discussions, capacity-building initiatives, and empowerment activities for CSOs, are planned. The child rights curriculum is being used in three universities in the region, with public lectures and capacity-building activities for both preservice and in-service professionals underway. Advocacy efforts supported by the One UN JA, included participation in the 16 Days of Activism  campaign, raised awareness of domestic violence issues. Awareness sessions on gender-based violence were conducted for Roma women, with plans to continue these efforts in 2024. In the second phase of the Programme substantial progress was made in enhancing the environment for people living with HIV (PLHIV) on the left bank of the Nistru River. Building on previous policy changes, the project focused on community-led monitoring (CLM) to ensure these changes were effectively implemented. The Programme continued its work with media, by organising human rights media brunches to build the capacity of media representatives from both banks of the Nistru River. These events, attended by over 25 journalists and media professionals, covered topics such as the access of Ukrainian refugees with disabilities to healthcare and the human rights of older persons. Through these sessions, media representatives gained deeper insights into the challenges faced by marginalized groups and the importance of adopting a human rights-based approach in their reporting. The Programme supported human rights awareness-raising initiatives in the Transnistrian region to promote the rights of marginalized groups. Key activities included public events on human-rights-compliant community-based services, on temporary protection status for Ukrainian refugees, an awareness event celebrating Roma rights, various campaign on HIV awareness and combating related stigma and a project creating human rights murals to symbolize social cohesion. These initiatives significantly enhanced knowledge and engagement on human rights issues among diverse communities. \n\nAdditionally, the mentoring programme, successfully launched in the first phase of the One UN JA, trained 36 women and 5 men in psycho-social skills and rights, empowering them to live free from stigma and violence. Monthly sessions on mental health, co-dependency, and overcoming violence created impactful experiences, exemplified by testimonials like Linda Anataciuc's.\n\nUpdate 2023:\nIn January 2023, the programme set out on an ambitious journey to strengthen security sector governance in Moldova under the \"Strengthening Security Sector Governance in Moldova\" (SSGM) Programme. Jointly implemented by a consortium of UN agenciesincluding IOM, OHCHR, UNAIDS, UNICEF, UNFPA, UNDP, and UNODCand coordinated by UNDP with Alexandru Stratulat at the helm, the initiative quickly established a comprehensive roadmap for change.\n\nAt the outset, the programme sought to create a more streamlined and effective approach. The Sustainable (Community) Development Platform (SCDP) was reactivated, and the number of Thematic Working Groups was reduced from three to two, a move that was designed to simplify coordination and enhance collaborative efforts. A roundtable in Tiraspol further bolstered these efforts by raising awareness about the importance of human rights-compliant community services.\n\nThe anticipated achievements soon began to materialize. Training sessions were organized to help NGO staff manage the increased workload and prevent burnoutan issue exacerbated by the ongoing Ukraine conflict. At the same time, capacity-building initiatives were launched for civil society organizations (CSOs) working with people living with HIV, pinpointing key areas for improving HIV services. The programme also extended its support to human rights defenders by offering specialized training in disability rights and establishing referral pathways for refugees with disabilities. Efforts to support vulnerable groups reached out to the Roma community through targeted consultations and cross-river cooperation, while temporary shelters and support services were provided for women who use drugs, as well as refugee women and children from Ukraine.\n\nIn a notable innovation, the programme introduced a new component focused on sexual and reproductive health education for young people in the Transnistrian region. This was complemented by the development of a Human Rights Framework (HRF) and the launch of new human rights education curricula aimed at combating stigma and discrimination among future medical professionals. Additional initiatives, such as media brunches and awareness-raising events, further amplified the impact by empowering communities with essential psychosocial skills and a robust understanding of their rights.\n\nThe volatile geopolitical situationmarked by the conflict in Ukrainecreated significant security concerns that hampered face-to-face interactions. Many activities had to be moved online, leading to a reduction in direct support and engagement with beneficiaries. The introduction of a new internal procurement system also caused delays, particularly affecting the implementation of One UN JA activities. Furthermore, overlapping roles between the Adjustment of the Normative Framework and Advocacy groups necessitated structural changes, and the Roadmap for Child Rights in the Transnistrian region, despite validation by de facto structures, struggled to gain approval due to the need for extensive amendments to existing regulations.\n\nDespite these unexpected hurdles, the programme demonstrated a remarkable capacity for adaptation. Its flexible approach allowed it to navigate security concerns and operational limitations, ensuring that progress continued even under challenging circumstances. In the end, both the expected outcomes and the unforeseen challenges collectively underscored the programmes resilience and commitment to promoting human rights, community development, and effective governance in Moldova.\n\nUpdate 2024:\nIn 2024, Sida remained a core donor to the Moldova 2030 SDG Partnership Fund  Phase II (MD200), which finances the One UN Joint Action to Strengthen Human Rights in the Transnistrian Region alongside other joint initiatives. Sida is the second-largest donor after Switzerland, with cumulative contributions reaching USD 2.5 million (approx. SEK 26.3 million) by the end of the reporting year. The Fund received USD 5.6 million in new contributions during 2024, bringing the total since inception to USD 7.45 million. Transfers to UN agencies amounted to USD 6.4 million in 2024, with cumulative disbursements of USD 7.37 million across ten UN entities. However, reported expenditures remained modest at USD 1.53 million in 2024, which corresponds to a delivery rate of 27 percent, reflecting the fact that most projects only entered their implementation phase during this year.\n\nWhile the project delivered strongly on its expected results in rights awareness, health services, and GBV response, the unexpected results included the success of digital treatment platforms, the recognition of Roma mediators by de facto authorities, and the projects demonstrated resilience in adapting to crises (energy insecurity, refugee influx).\n\nExpected Results Achieved:\n- Roma community mediators expanded and strengthened: Six mediators now operate across six localities, assisting 1,000 Roma people, including 284 Ukrainian refugees, in obtaining identity documents, securing property rights, accessing health and education, and benefiting from social protection schemes.\n- Gender-based violence services consolidated: The GBV trust line responded to 359 calls, opening 28 SOS cases, while three crisis rooms provided safe shelter and support to women who use drugs, sex workers, and refugees with children.\n\n- Improved HIV-related health services: Through community-led monitoring, systemic gaps were identified, leading to 250 healthcare workers being trained in rights-based, non-discriminatory communication and patient care.\n- Youth sexual and reproductive health advanced: 75 adolescents and 52 parents or caregivers participated in extracurricular sexuality education, while 22 health specialists were trained to deliver youth-friendly services.\n- Policy advocacy sustained: Civil society groups continued dialogue with de facto authorities on the Human Rights Framework, keeping human rights issues on the agenda despite political challenges.\n- Human rights awareness expanded: Public campaigns, including International Roma Day, HIV stigma reduction initiatives, and the 16 Days of Activism, directly engaged over 1,500 people across both banks of the Nistru River.\n\nUnexpected \u002F Innovative Results:\n- Digital innovation in health services: The launch of the VADO-ASSIST platform introduced video-assisted treatment for opioid dependence. This innovation enabled 68 patients to receive care under remote supervision of 14 medical staff, reducing stigma and logistical barriers to treatment.\n- Rapid adaptability to energy insecurity and refugee needs: When the looming gas crisis created fear of service disruptions, the programme adjusted activities to ensure continuity, for example by securing shelters and trust line operations, and by expanding mediators work to support refugees from Ukraine.\n- Cross-river trust-building effects: Roma mediators and civil society groups engaged in joint advocacy and training across both banks of the Nistru River, unexpectedly generating new levels of recognition from de facto authorities who began to regard mediators as trusted partners.\n- Strengthened community voice: Grassroots advocacy by Roma activists led to the drafting of a proposal for formal recognition of Roma mediators in Transnistrian regulationsan outcome not initially foreseen in the project design.\n\nBased on the 2024 progress report, the project provided three short impact to illustrate concrete results of the One UN Joint Action in Transnistria:\n\n1. A Roma Family gains legal identity\n\nIn Dubasari, a Roma family who had lived undocumented for years received long-awaited identity papers with the help of a newly trained Roma mediator. The parents were finally able to register their children for school and access social assistance. For the first time, the family could claim basic rights such as healthcare and child benefits. This case highlights how a relatively small intervention, legal support from a community mediator, can break a cycle of exclusion affecting multiple generations.\n\n2. A survivor of Gender-Based Violence finds safety\n\nA young woman from Tiraspol, fleeing domestic violence with her child, reached out to the GBV trust line. Her case was classified as urgent and referred to a crisis room supported by the project. There, she received safe shelter, psychosocial support, and medical care. The intervention not only ensured her immediate safety but also helped her begin the process of rebuilding her life with dignity and security. This case underscores the life-saving role of Sida-funded GBV services in an environment where such protection is otherwise scarce.\n\n3. A young refugee accesses Sexual and Reproductive Health services\n\nA 17-year-old Ukrainian refugee living in Transnistria joined an extracurricular programme on comprehensive sexuality education introduced by the project. Initially hesitant, she later described the sessions as empowering, particularly because they provided practical knowledge on reproductive health and gender equality in a supportive environment. The programme helped her connect with youth-friendly health services, ensuring that refugee adolescents are not left behind in accessing information and care.\n\nThese stories show that Sidas support is not only contributing to policy and institutional reform but is also transforming the lives of individuals and families in highly vulnerable situations.\n\nUpdate 2025: \nSida amended the contribution to the One UN Joint Action in Transnistria and extend the implementation period by 13 months, until 31 December 2026. \nDuring 2025, the programme demonstrated strong progress across all main outcome areas despite operating in a volatile and politically sensitive context. Key achievements included expansion of Roma community mediation services, strengthening of gender-based violence response mechanisms through crisis rooms and trust lines, improved access to rights-based health services, and increased awareness among youth regarding sexual and reproductive health and rights. The programme also demonstrated flexibility during the 2024-2025 energy crisis by reallocating resources to support vulnerable groups and maintain essential services.\nAccess to rights and services for vulnerable groups improved, with 8 CSOs supporting more than 340 vulnerable individuals through advocacy initiatives, digital information tools and pilot services for persons with severe disabilities and children with autism.\nAdolescents gained greater access to rights-based sexual and reproductive health information and services, as trained providers delivered consultations and educational activities to over 4,400 adolescents, while an additional 1,700 adolescents received individual consultations and approximately 2,500 young people participated in awareness sessions.\nEducation and health institutions strengthened their capacity to address adolescent health challenges, with 86 education specialists trained to respond to adolescent pregnancy and 22 health professionals improving their competencies in adolescent-friendly and rights-based healthcare services.\nPeople living with HIV became more empowered to claim their rights and address gender-based violence, with 209 women living with HIV strengthening their ability to recognize, report and manage violence, while participating more actively in advocacy and policy dialogue.\nChild protection actors improved their readiness to provide child-friendly justice services, with assessments and advocacy efforts creating a pathway for introducing the Barnahus model and strengthening interagency cooperation in responding to violence against children.\nHuman rights awareness and social cohesion increased across the region, with over 1,500 direct beneficiaries participating in human rights campaigns and events, more than 240 participants engaged in Roma rights initiatives and 19 media professionals improving human-rights-based reporting, resulting in the publication of four human
