The institute is today a centre of excellence producing research of global relevance, such as child and maternal health, including reproductive health.\n\nicddr,b provides unique opportunities to address health challenges in low-income settings through low-cost, practical, lifesaving solutions. The track record includes the oral rehydration solution (ORS), zinc for diarrhoea treatment and tetanus vaccines for newborns, which have saved and continue to save millions of lives worldwide. icddr,b has the capacity to undertake a full spectrum of research spanning population-based studies and demographic surveillance, large-scale clinical trials, hospital-based clinical studies, and laboratory research.\n\nicddr,b has recently had some turbulent years with concerns of both organizational nature and regarding research sustainability, especially with regards to the regrowth of researchers at the senior level. These challenges are being addressed in different ways and the institute is currently at a transitional stage. icddr,b’s\n\nStrategic Plan 2015-2018 reinforces icddr,b’s legacy of focusing not just on discovery, but also on strengthening the organisation’s ability to shape policy at the national, regional and global level, as well as advocating for the use of highly effective products and evidence-based discoveries. icddr,b reinforces these efforts through publishing and South-South collaboration, training and capacity-building, and continues to build a cadre of public health experts and capacities for low- and middle-income countries. In addition, strategic national, regional and global partnerships tap into the complementary assets of other organisations—academic, scientific, NGOs and private sector—to maximise the influence and reach of icddr,b’s work.\n\nThere are five key areas in which icddr,b has a strong track record and expertise (points 1-5 below) and two which are highly relevant to Bangladesh and where icddr,b would like to develop greater expertise (points 6-7 below):\n\n1Reduce maternal and neonatal mortality\n2Prevent and treat maternal and childhood malnutrition \n3Control enteric and respiratory infections \n4Detect and control emerging and re-emerging infectious diseases\n5Achieve universal health coverage\n6Non Communicable Diseases\n7Climate Change\n\nAn important value of the Sida research support is the earmarking of research project funding to younger scientists and a possibility to plan a career development program of several years in a row. Stödet från Sida är programstöd till kapacitetsökning hos unga forskare i enlighet med deras Strategiska Plan 2015-2018, vilket är en förutsättning för den flexibilitet som krävs för att kunna anpassa aktiviteterna efter behov och erfarenhet. Sida är för närvarande den enda givaren som stödjer denna del av strategin specifikt. \n\nIcddr,b har målsättningen att utveckla lösningar inom hälsa som är kostnadseffektiva och realistiska för fattiga människor som lever i låginkomstmiljöer. Icddr,b är idag ett framstående center för forskning av global relevans, exempelvis barn- och mödravård samt reproduktiv hälsa. De arbetar utifrån ett starkt rättighetsperspektiv och ställer den fattiga människan i centrum för sitt arbete. De planerar och investerar i långsiktiga hälsoinsatser och har en rådgivande funktion inom bla WHO. \n\nicddr,b har utvecklat många processer och produkter av stor betydelse för hälsa i låginkomstmiljöer, till exempel ett världsledande koleravaccin och vätskeersättningen ORS. Nyligen har de tagit fram en enkel-dos av koleravaccinet, en lågkostnads-luftpump för småbarn med lunginflammation och stelkrampsvaccin för nyfödda. \n\nMålsättningarna i icddr,b's Strategiska Plan 2015-2018 är att bli ett världsledande center inom forskning på global hälsa, och att bygga en bättre och effektivare finansiellt hållbar institution.  \n\nDe fem strategiska prioriteringarna är:\n1. Minska mödra- och spädbarnsdödlighet\n2. Förebygga och behandla undernäring hos mödrar och barn\n3. Kontroll av enteriska- och luftvägsinfektioner\n4. Upptäcka och förebygga nya och återkommande infektionssjukdomar\n5. Uppnå allmän\u002Funiversell sjukvårdsförsäkring \n\nUtöver dessa fem områden, där icddr,b är internationellt konkurrenskraftiga, vill man bygga upp starkare kapacitet inom områdena: hälsoeffekter av klimatförändringar, samt förebyggande och behandling av icke smittsamma sjukdomar. I Bangladesh har man en unik möjlighet att studera just kopplingen mellan hälsa och klimatförändringar. \n\nDet svenska stödet är inte stort jämfört med de riktade forskningsanslag som centret attraherar, men det svenska forskningsstödet ger ett stort mervärde och kan anses katalytiskt. Ägarskapet för insatsen ligger i linje med centrets strategiska plan. Då majoriteten av forskningsmedel är öronmärkta av finansiärerna är det föreslagna svenska stödet av största vikt för centret då det möjliggör långsiktiga satsningar som följer centrets egna prioriteringar. Fokus ligger på återväxten av framtida starka forskningsledare på centret – något som dels möjliggör icddr,b’s framtid, dels stärker forskningsområdet i sig. Sida kan göra störst skillnad genom ett flerårigt stöd som möjliggör en satsning på unga lovande forskare, vilka annars har svårt att få extern finansiering i konkurrens med etablerade seniora forskare. Dessa unga lovande forskare välj ut i konkurrens och måste vara anställda som forskare vid icddr,b och vara under 40 år. Stödet kommer även att gå till etableringen av icddr,b’s genomikfacilitet vilken initierades med svenskt stöd under 2015-2016. \n\nSida har gett finansiellt stöd till icddr,b sedan 1981 och har erfarenheten att centret har god kapacitet att uppnå sina mål.",1,{"narrative":222,"type":225},[223],{"value":224},"Research support\n\nThe main objective of the ongoing contribution is the development of skills and competencies of scientific staff; with a special focus on local junior researchers at mid-level, through a capacity training program. The participants are selected in competition and will go through a career development program including running their own research projects. In 2018 another component was added, for the study of an improved treatment of small children with severe pneumonia in Ethiopia. The method has been developed at icddr,b and is used at the hospital of icddr,b. The long-term purpose is to change the global guidelines of this treatment. \n\nStrategic programme support\n\nThis component is a continuation of last year's strategic funding to enable icddr,b to enhance their activities of particular relevance to public health interventions that are linked to Water, sanitation and hygiene (WASH), Anti-microbial resistance (AMR), Climate change-related health impacts and non-communicable diseases (NCDs) in urban slum populations - with global added value.\n\nExpected outputs are: A series of theoretically based indicators that bear on vulnerability and wellbeing of the urban poor, related directly and indirectly to the topics of WASH, AMR, NCDs, CCH and COVID-19; A series of measurement tools, equipment, and techniques to support the measurement of risk factors, exposures, and health outcomes in the urban poor; and Hardware and software to support the collection curation and communication of research data which will support project management within icddr,b and as part of broader South-South collaborations. international health research institution located in Dhaka, dedicated to addressing health challenges that disproportionately affect people living in poverty in Bangladesh and other low- and middle-income countries.\n\nSida has supported icddr,b since 1981 and the institute has evolved considerably over time broadening its focus from cholera and diarrhoeal diseases to include many health challenges affecting vulnerable and neglected populations. The institute is today a centre of excellence producing research of global relevance, such as child and maternal health, including reproductive health. icddr,b provides unique opportunities to address such health challenges in low-income settings through low-cost, practical, lifesaving solutions and has the capacity to undertake a full spectrum of research spanning population-based studies and demographic surveillance, large-scale clinical trials, hospital-based clinical studies, and laboratory research. icddr,b is committed to translating research results into evidence-based policy and action.\n\nSida's support in the current agreement period (2016-2025) included two components financed by the Swedish strategies for research cooperation and research in development cooperation 2015-2021 and global development cooperation in sustainable development 2018-2022 (and their later renewed versions), respectively. While the first component focused on research, support to young researchers and institutional capacity building, the latter was added to the contribution in and 2019 had urban health challenges as primary focus area.\n\nExamples of achievements during previous and the recently ended funding period have been presented in an article on Sida's external website in 2023: Sidas forskningssamarbeten i Bangladesh tar fram billiga och enkla lösningar som räddar liv (https:\u002F\u002Fwww.sida.se\u002Fsida-i-varlden\u002Flander-och-regioner\u002Fbangladesh\u002Fsidas-forskningssamarbeten-i-bangladesh-tar-fram-billiga-och-enkla-losningar-som-raddar-liv).\n\nBecause Sida's support to icddr,b ended in 2024 and currently no new funding period is planned, some of the key results of the more than 40-year long partnership between Sida and icddr,b are listed below. icddr,b's solutions have saved and continue to save millions of lives worldwide, including the lives of newborns, children under five, mothers, migrants and refugees.\n\n> icddr,b has played a pivotal role in the development and testing of an oral cholera vaccine (OCV) in collaboration with Swedish researchers and pharmaceutical industry. OCV is today used both preventively and reactively, in endemic communities, outbreak zones and humanitarian and refugee settings.\n\n> Development and testing of oral rehydration therapy, a simple and cheap life-saving therapeutic intervention to treat electrolyte imbalance caused by diarrhoea across the world.\n\n> Evidence from icddr,b showed that zinc treatment reduces duration, severity and recurrence of diarrhoea and led revised WHO and UNICEF guidelines for the management of childhood diarrhoea.\n\n> Clinical trials proved that maternal tetanus vaccination drastically reduces deaths from neonatal tetanus and contributes to neonatal tetanus elimination.\n\n> Development of a low-cost bubble continuous positive airway pressure (bCPAP) device to deliver oxygen to young children with pneumonia. The device is made from simple, locally available materials, and has the potential to bring bCPAP therapy to a wide range of facilities in resource-poor settings. Clinical trials at icddr,b Dhaka Hospital and 12 secondary hospitals in Ethiopia (in cooperation with Armauer Hansen Research Institute and directly financed by Sida) demonstrated that treatment of young children with severe pneumonia and hypoxemia with bCPAP oxygen therapy reduced death rates by 70%, led to fewer treatment failures and shortened hospital stays compared to current standard WHO-recommended therapy. Based on this evidence, the Ministries of Health in Ethiopia, Malawi, and Nigeria, with support from the European Union (OPT-bCPAP project financed by Global EDCTP3 with partners at Karolinska Institutet and Uppsala University), are now scaling up bCPAP in 37 hospitals across Africa.\n\n> Development and testing of a biodegradable absorbent birthing mat for home deliveries that warns of dangerous blood loss and indicates that mothers need specialist medical care. Along with the drug misoprostol to limit bleeding and umbilical cord cleaning tools, the mat has been incorporated into an innovative safe delivery kit that now is in widespread use in Bangladesh.\n\n> The icddr,b Genome Center was established with the support from Sida in 2015 and has developed into a leading facility for genomic research in Bangladesh. The center is today self-sufficient and contributes among other things to pathogen surveillance, research concerning the genomics of antimicrobial resistance and cancer diagnostics. During the COVID-19 pandemic, the center has contributed to the SARS-CoV-2 variant surveillance in Bangladesh.\n\nIn addition to scientific outcomes, icddr,b has invested in the training of young scientists and strengthening institutional capacity. Through a Sida-supported special training programme, young promising researchers got the chance to pursue their own research projects as principal investigators. icddr,b has regarded this support as truly catalytic as it allows the growth of the next generation of researchers. Research projects were in the area of health research but some included climate, environment and gender aspects, contributing to strengthening icddr,b's capacity in these areas.\n\nThe component Urban Health Coverage model for scale up through south-south partnerships has supported improved management of icddr,b's asset of health data including icddr,b's Health and Demographic Surveillance Systems (HDSS), development of methodology to enhance data quality and a new set of indicators for improved understanding of vulnerabilities and risks in urban populations (and subgroups) that are accentuated during epidemics. The previously established HDSS contribute to improved management of public health crises through surveillance workers following up experiences and development at household level and record demographic events such as pregnancies, births, deaths, and migrations. HDSS help to understand how social, environmental and demographic factors interact over time to produce health outcomes and monitor national and global health goals. \n\nThe component has furthermore enabled the establishment of the HDSS Asian Network for collaborative work including Bangladesh, India, Indonesia, Malaysia, and Vietnam. Another example of strengthened south-south partnership is the collaboration of icddr,b with ten HDSS groups in Africa and South Asia for implementing the project \"HDSS-based Mortality Surveillance in Africa and South Asia\".\n\nExamples of evidence that have been generated include the understanding of irrational use of antibiotics and knowledge levels about antibiotic use and antimicrobial resistance (AMR) among informal care providers and the general population. Another piece of evidence relates to inequities in access to social security systems between rural and urban areas.\n\nFurthermore, the Urban Health Coverage component contributed to the retention of staff and strengthening of staff capacity in both primary, secondary and systematic research including data analyses and reviews during the COVID-19 pandemic in
