TDR helps facilitate, support and influence efforts to combat diseases of poverty, with the mission to \"support effective and innovative global health research, through strengthening the research capacity of disease-affected countries, and promoting the translation of evidence into interventions that reduce the burden of infectious diseases and build resilience in the most vulnerable populations\". TDR main areas are i) Research for policy; ii) Research for implementation; iii) Research for innovation; and iv) Research for integrated approaches.       \nSida\u002FSweden is an as long-standing supporter of HRP, even since a couple of years before its creation in 1972. As such, Sida is equally familiar with the HRP's achievements. HRP is the main instrument within the United Nations system for research in human reproduction, bringing together policy-makers, scientists, health care providers, clinicians, consumers and community representatives to identify and address priorities for research to improve sexual and reproductive health. The mission of HRP is \"the attainment of all people of the highest possible level of sexual and reproductive health. HRP strives for a world where all womens and mens rights to enjoy sexual and reproductive health are promoted and protected, and all women and men, including adolescents and those who are underserved and marginalized, have access to sexual and reproductive health information and services\".  \nAHPSR is a younger programme than TDR and HRP and recently celebrated its 20th anniversary. Sweden was one of the drivers of the creation of AHPSR and has been actively involved since then, leading to good insights into their activities. AHPSR work to improve the health of those in low- and middle-income countries by supporting the generation and use of evidence that strengthens health systems. Together with organizations around the world, the AHPSR aims to i) provide a unique forum for the health policy and systems research community; ii) support institutional capacity for the conduct and uptake of health policy and systems research; iii) stimulate the generation of knowledge and innovations to nurture learning and resilience in health systems; and iv) increase the demand for and use of knowledge for strengthening health systems.\n \nHRP and TDR are special programmes of WHO which gives them an independent status in relation to WHO. They are co-sponsored by a number of UN-agencies (UNICEF, UNDP, World Bank and WHO are co-sponsors of both TDR and HRP, and additionally, UNFPA is co-sponsor of HRP). As a WHO-hosted partnership, AHPSR is also guaranteed its independence. The difference between the two set-ups used to be the co-sponsorship of the special programmes, although today the definitions are being discussed at WHO. The future format of AHPSR is described later in this appraisal.\nWhen the special programmes were created in the early 1970s WHO did not actively engage in research, but as a number of member states could see the importance of producing research of relevance (in this case tropical diseases and human reproduction), TDR and HRP were created. Over the years, the importance of producing evidence to feed into policies and guidelines was increasingly recognized and other UN agencies joined the effort as co-sponsors.\nSimilarly, the areas of health policy and systems research were neglected and in the late 1990s a group of global health leaders, including senior scientists, policy makers and representatives of various agencies and programs with a stake in health policy and systems research agreed on the need to create a body that would address these areas, and AHPSR was created as a hosted partnership at WHO.\nBeing based at WHO and having a close interaction with the co-sponsors provides the research programmes with a unique opportunity to accelerate translation of research results into policies and guidelines. Through country and regional offices and their close relation to a country's Ministry of Health, the possibility for implementation at this level is also improved. As WHO is now putting more emphasis into the country and regional offices, this is expected to play an even more important role in the coming years. The importance of research and evidence is even more clear now that WHO has assigned a first Chief Scientist and a Science Division, and one can foresee a closer interaction between the research programmes and WHO at all levels.\nOver the years, as the world changes, the areas of research addressed as well as the ways to address the need of evidence and implementation have changed in the three programmes. The research programmes show an embedded ability to adapt their activities to changes in global health and to take leading roles in the global research agenda.\nThere are still pertinent questions to be addressed in the programmes respective areas. For example, there is presently a strong focus on implementation research and interaction with decision-makers, as well as strengthening research capacity in LMICs and ensuring that high quality research is done in countries. \nAs the bulk of Sida's support to the programmes is given as undesignated funding, the results of the upcoming phase are expected to contribute to the attainment of the respective AHPSR's work will focus on five areas during this strategy period: 1) reforming health systems and services to achieve universal health coverage through a primary health care approach; 2) improving health through digital transformation; 3) preventing premature death and disability by tackling the risks and determinants of noncommunicable diseases; 4) building health systems resilience and realizing co-benefits in response to the climate crisis; and 5) applying a systems approach to health emergency preparedness and response.\n\nResults documented by AHPSR during 2024 include:\n\n- AHPSR supported 68 grants across 43 countries and territories during 2024;\n\n- AHPSR produced 46 peer-reviewed journal articles. 41% of lead authors were women and 51% of journal articles had lead authors from LMICs. Examples  of articles include:\n\n     * Future proofing health in response to climate change and rapid urbanisation in Africa by Monika Kamkuemah, Olalekan Ayo-Yusuf and Tolu Oni in The BMJ \n\n     * Engaging non-state providers towards PHC in South Asia: considerations for policymakers by Zubin Cyrus Shroff, Anas Ismail, Kaosar Afsana, Manuj Weerasinghe and Krishna D. Rao in The Lancet Regional Health Southeast Asia.\n\n     * Tying health taxes to health promotion is popular and effective in Thailand by Viroj Tangcharoensathien, Prakit Vathesatogkit and Supakorn Buasai in The BMJ\n\n     * Prepared for the polycrisis? The need for complexity science and systems thinking to address global and national evidence gaps by Aku Kwamie, Sara Causevic, Goran Tomson, Ali Sie, Rainer Sauerborn, Kumanan Rasanathan and Ole Petter Ottersen in BMJ Global Health\n\n- Cumulatively (as of 2024), the Alliance has supported over 1100 publications, which have been cited around 65 000 times across academic and policy literature. In 2024 alone, Alliance-supported publications were cited nearly 12 000 times,  underscoring the influential impact and significance of our work. The growing citation base is more than a measure of reach  it shows how ideas, approaches and studies championed by the Alliance have been used by the community.\n\n- AHPSR co-sponsored the 8th Global Symposium on Health Systems Research in Nagasaki in November 2024. This commitment included supporting participation of 58 persons, organizing 5 satellite sessions, hosting 3 organized sessions on health taxes, and the links between health and climate change, and leading 2 capacity strengthening sessions with a focus on research communication and political reform.\n\n- The Ethiopian Ministry of Health created a dedicated unit for evidence-informed policy-making  with support from the Alliance and the WHO Country Office for Ethiopia (2024 Annual Report, p 5).\n\n- In francophone Africa, six AHPSR fellows trained 149 graduate students in five countries, while also engaging institutional leadership to embed health policy and systems research into academic offerings. These efforts represent a shift from one-off training to deeper, more sustainable institutional capacity strengthening  helping to ensure local ownership, continuity, and long-term impact in health systems research and policy. (2024 Annual\nReport, p. 25)\n\n- AHPSR identifies a critical need to develop the evidence base on the intended and unintended consequences of adopting and implementing digital interventions in health systems in low- and middle-income countries (LMICs). As such, AHPSR, in collaboration with the WHO Department of Digital Health and Innovations, is supporting ten research teams in eight countries to better understand the role of digital solutions in strengthening health systems in LMICs. In Bhutan, for example, AHPSR and local partners investigated the impact of Bhutan Vaccine System on vaccination efficiency and empowerment of Bhutan's citizens. The study's findings have subsequently had a significant impact on Bhutan's health system. Firstly, the research identified gaps in digital health infrastructure, prompting the Ministry of Health to plan solar power backup solutions for facilities without reliable electricity and improve access to digital tools for healthcare workers. Secondly, key findings were shared with the Electronic Patient Information System (ePIS) team, influencing the ePIS project to prioritize patient-centric applications and address issues such as internet and electricity reliability, as well as user training. Lastly, the study contributed to the revision of Bhutans National eHealth Strategy and Costed Action Plan, with the research team providing ongoing technical assistance from WHO. (sources: 19\u002F001414-53 and 2024 Annual Report, p. 9).\n__________________________________________________________________________________________\n\nHRP\n\nSpecific results from the HRP include:\n\n- During 2024, HRP joined other Sida-partners (SVRI-Sexual Violence Research Initiative and UNICEF) to better understand and counteract violence against women and violence against children (see HRP 2024 Annual Report, p 9). The partners carried out a global participatory process to identify research priorities that address the links between violence against children and violence against women. The identification of research priorities advances understanding of violence and supports the efficient use of limited research funds. Furthermore, the research priorities will inform the implementation of the multi-agency RESPECT Women and INSPIRE frameworks; support the efforts of UNICEF to respond to the gender dimensions of violence against children; strengthen WHO's work addressing violence against women and violence against children; and guide the SVRI grant making strategy. The priorities were determined in an inclusive manner, including strong participation from low- and middle-income countries which have historically lacked representation (Source: Intersections between violence against children and violence against women: global research priorities, WHO 2024)\n\n- During 2024, 494 individuals were trained with support from the HRP Alliance. Of these persons, 451 took part in short courses (270 female and 181 male). Courses were intended to strengthen knowledge translation and engagement of policy and decision-makers (HRP 2024 Annual Report, p.43).\n\n- Twenty-two PhD students (11 female, 11 male) completed PhD programmes at HRP Alliance hubs in 2024. Twenty-one masters students (16 female, 5 male) completed their degrees as well (HRP 2024 Annual Report, p47).\n\n- During 2024, 200 scientific papers were published with support from HRP. 59% of first authors were women, and 48% of first author institutions were in LMICs. \n\n- Four WHO normative guidelines were produced with support from HRP. These include \"WHO recommendations on self-care interventions: Making over-the-counter emergency contraceptive pills available without a prescription\" and \"WHO recommendations on the assessment of postpartum blood loss and use of a treatment bundle for postpartum haemorrhage\".\n\n- The new WHO Model list of Essential Medicines (EML) The selection and use of essential medicines 2025 was released in September 2025 (https:\u002F\u002Fwww.who.int\u002Fpublications\u002Fi\u002Fitem\u002FB09474). HRP supported the updated list, not least through the removal of a caveat and an asterisk alongside the listing of mifepristone-misoprostol that has existed in the essential medicines list since 2005. The caveat indicated that mifepristone-misoprostol should only be used for safe abortion where legally permitted and culturally acceptable. It is now recognized that the caveat was unrelated to scientific evidence, and redundant given that all medicines listed in the EML are assumed to be provided within relevant legal and regulatory frameworks.\n\n- the EASIER Project scaled-up use of subcutaneous depomedroxyprogesterone acetate (DMPA-SC) self-injection through implementation research. This  research led to policy and programme changes in Burkina Faso, Ghana and Kenya. Mozambique, Ethiopia and Madagascar are also expected to adapt and implement the protocol soon. DMPA-SC is a lower-dose, injectable contraceptive that women and adolescent girls can learn to administer  themselves, offering protection against pregnancy for three months. It has significant potential to reduce unmet need for contraception because it can be administered by health workers and women themselves, improving access to contraception, thus contributing to the SDGs and universal health coverage. (HRP 2024 Annual Report)\n\n- engaging men and boys in SRHR in a way that challenges harmful masculinities is important for improving SRHR amongst both women and men. To address this gap, a global research priority-setting exercise on masculinities and SRHR was published in 2024. The exercise forged a collaborative research agenda emphasizing four key areas: gender-transformative approaches to mens and boys engagement in SRHR; applied research to deliver services addressing diversity in SRHR among men and women and to generate gender equality; research designs to support participation of target audiences and reach to policy-makers; and research addressing the priorities of people in low-income and middle-income countries. The masculinities and SRHR shared research agenda was published and launched with Sida-partners UNFPA and MenEngage Alliance. (HRP 2024 Annual Report)\n\n- The recommendations contained in the HRP\u002FWHO abortion care guideline (in particular the law and policy recommendations) have been used to guide national policy reform processes in Ethiopia and Mozambique. (HRP 2024 Annual Report)\n\n- HRP, in collaboration with University of Birmingham (United Kingdom), previously tested an approach for early detection of postpartum  haemorrhage (PPH) with a calibrated obstetric drape and use of a care bundle for treatment of PPH. The approach resulted in dramatic improvements in maternal outcomes  a 60% reduction in severe PPH ( 1000 ml), laparotomy for bleeding or maternal death from bleeding. New evidence from this trial shows that the use of the PPH bundle approach is cost-effective and even cost-saving when the price of a calibrated  obstetric drape, which accurately measures blood loss during childbirth, is below US$ 1. The global community and manufacturers are stepping up efforts and local production of calibrated drape to implement the PPH  bundle approach at scale in several high-burden countries. (HRP 2024 Annual Report)\n\n- HRP and WHO produced a new brief on contraception and women\u002Fyouth\u002Fadolescent empowerment and health  (https:\u002F\u002Fwww.who.int\u002Fpublications\u002Fi\u002Fitem\u002FB09437). The brief finds that contraception empowers women and positively impacts their overall health and well-being by reducing health risks, contributing to a healthier life and conferring greater agency. Specifically, contraceptive use reduces health risks associated with high-risk pregnancies, such as pre-eclampsia and endometrial, ovarian and cervical cancers;  contributes to a healthier life by improving anaemia and menstrual health, reducing the size of leiomyoma and dysfunctional bleeding and lowering pain associated with endometriosis; decreases depressive symptoms among women with pre-existing mental disorders and has a protective effect against anxiety in women; and empowers women by contributing to their socioeconomic status and wellbeing. \n\n- HRP is partnering to integrate gender-based violence prevention into health programming in conflict areas, for example technical guidance in humanitarian settings in Tigray (Ethiopia), Coxs Bazaar (Bangladesh), BAY States (Nigeria), Cameroon, Colombia, Libya, Moldova, Morocco, Romania, Somalia, South Sudan and Ukraine.\n___________________________________________________________________________________________\n\nTDR\n\nResults from TDR during January 2024 - June 2025 include:\n\n- TDR's achievements in 2024 included 21 instances when research projects supported by TDR informed policy and\u002For practice in countries to address infectious diseases of poverty, and 6 instances when TDR tools and reports were used to inform policy and\u002For practice of global\u002Fregional stakeholders or major funding agencies (TDR 2024 Annual Report, p. 6).\n\n- TDR supported research and capacity strengthening of 281 scientists through its postgraduate training scheme, impact grants for regional priorities, clinical research and development fellowship, and SORT IT program (TDR 2024 Annual Report, p. 6).\n\n- 50% of TDR's total research grant\u002Fcontract amounts were awarded to women in 2024, and 44% of peer reviewed publications supported by TDR have women as first author. 71% of peer reviewed publications supported by TDR have first authors from disease-endemic countries (TDR 2024 Annual Report, p. 6).\n\n- TDR's implementation research supported the roll-out of paediatric praziquantel for schistosomiasis, malaria vaccine, and moxidectin for onchocerciasis (TDR 2024 Annual Report)\n\n- Ghana's 2024 approval of moxidectin for the treatment of river blindness (onchocerciasis)in children and adults is partly the result of TDR's long term work to find solutions to neglected diseases of poverty. In 2023 at least 249.5 million people required preventive treatment against onchocerciasis, and the Global Burden of Disease Study estimated in 2017 that 14.6 million of the infected people already had skin disease and 1.15 million had vision loss (https:\u002F\u002Fwww.who.int\u002Fnews-room\u002Ffact-sheets\u002Fdetail\u002Fonchocerciasis). Moxidectin creates, for the first time, an opportunity to eliminate river blindness. Moxidectin development was initiated in the late 1990s by TDR in consultation with the Onchocerciasis Control Programme in West Africa (OCP) and the African Programme for Onchocerciasis Control (APOC). With funding from TDR and APOC donors, clinical studies were conducted as a collaboration between TDR, researchers in onchocerciasis endemic and other countries and WHO country offices. TDR, as part of the Access and Delivery Partnership (ADP), is currently collaborating with the Ghana Health Service, the University of Health and Allied Sciences in Ghana, the Bruyère Research Institute in Canada, Medicines Development for Global Health (MDGH) and disease-endemic communities to evaluate the acceptability and feasibility of moxidectin distribution through community-based mass drug administration (MDA). The results of this implementation research programme will further inform WHO and endemic country decisions on the role of moxidectin in onchocerciasis elimination strategies. This work is an example of TDRs engagement and collaboration with a wide range of partners to support implementation of new products and innovations (Source: TDR 2024 Annual Report, p8; https:\u002F\u002Ftdr.who.int\u002Fnewsroom\u002Fnews\u002Fitem\u002F12-12-2024-moxidectin-approved-by-the-ghana-food-and-drugs-authority-a-new-milestone-in-the-fight-against-river-blindness)\n\n- Implementation research supporting the AMR Global Action Plan, including evidence that the vast majority of 75 AMR research projects (79%) have influenced policy and practice (TDR 2024 Annual Report, p 26). Nine published studies that resulted from a SORT IT training cycle in Ghana were assessed for impact. The findings were recently published in a Special Issue of Tropical Medicine and Infectious Disease (2025, 10(11)). Impacts were identified at the Ghana Environmental Protection Agency (seafood industry effluent treatment), the National Technical Working Group on Water (water safety plan scaled up and implemented), and several hospitals (source: https:\u002F\u002Ftdr.who.int\u002Fnewsroom\u002Fnews\u002Fitem\u002F31-10-2025-research-on-amr-and-one-health-influence-health-policy-and-practice-in-ghana)\n\n- TDR research on Tuberculosis-caused disabilities directly contributed to WHO policy on TB-associated disability (TDR 2024 Annual Report, p 11).\n\n- New evidence on strategies to reduce impact of climate change on vector-borne diseases such as dengue, Zika, Chikungunya, yellow fever, meningitis and cholera (TDR 2024 Annual Report, p 17).\n\n- TDR-supported studies on short, all-oral regimens for drug-resistant tuberculosis have shown high treatment success rates and prompted national policy changes in the?Dominican Republic?and Ecuador, who have integrated the novel regimens into routine care for drug-resistant TB patients (Sources: https:\u002F\u002Ftdr.who.int\u002Fnewsroom\u002Fnews\u002Fitem\u002F20-03-2025-supporting-policy-makers-tackling-drug-resistant-tb and https:\u002F\u002Fbmcinfectdis.biomedcentral.com\u002Farticles\u002F10.1186\u002Fs12879-024-10417-w)\n\nIt was noted that the proportion of funds allocated to disease endemic countries was lower in 2024 (66%) than in previous years (79-88%) (see TDR 2024 Annual Report, p 6).
