Handicap International/Humanity and Inclusion (HI) har inkommit till Sida med ansökan om stöd om totalt 37 miljoner kronor för ett prjektet "Strenghtening physical rehabilitation services in the North-East of Syria" för perioden maj 2020 till april 2022. Projektet syftar till att öka tillgången till fysisk rehabilitering och proteser för människor drabbade av konflikt i nordöstra Syrien. HI arbetar sedan tidigare med sjukhus och hälsomottagningar i tre governorat i nordöstra Syrien (Hasakeh, Raqqa and Der Ezzor) och de kommer att ge 3900 personer skräddarsydda rehabiliteringsprogram som inkluderar bl.a. sjukgymnastik, tillhandahållande av hjälpmedel såsom proteser, psyko-socialt stöd etc. De kommer även att arbeta med familjemedlemmar för att ge dem stöd och öka deras kunskap, utbilda hälsopersonal i fysisk rehabilitering och psyko-socialt stöd samt arbeta med intern och extern remiterring till relevanta instanser. Sida är den enda givaren till detta projekt och finansierar hela projektkostnaden om 37 miljoner SEK. Controller har medverkat i bedömning av detta projekt och framtagande av avtal och beslut.

Resultat
Throughout the project, HI successfully implemented a comprehensive approach that included assessments and capacity building for implementing partners and health facilities in North East Syria. The implementation of the Rehabilitation Management System (RMS) enhanced the quality management of rehabilitation services by evaluating against established references and standards, aligned with the 6 WHO building blocks for health system strengthening. HI organized inclusive Training of Trainers (ToT) sessions and conducted 80 Gender and Development (GAD) awareness sessions to partner staff. To ensure heightened visibility of health facilities, 32 billboards were strategically installed across the Governorates of Raqqa, Hassakeh, and Menbij. In Hassakeh National Hospital, HI provided formal training on distinguishing between inflammatory and mechanical back pain, along with Mental Health and Psychosocial Support (MHPSS) in individual counseling. These efforts were coupled with consistent on-job individual coaching and supervision. For the mobile teams including the ones in Al Hol camp, 10 formal trainings took place in 2022, complemented by ongoing on-job coaching. Formal training sessions provided to local partner staff covered a spectrum of 21 physical rehabilitation topics, including Basics of Prosthetic and Orthotic care. Additionally, MHPSS support was extended to staff, covering a range of topics such as MHPSS humanitarian principles, PSS IASC guidelines, addressing specific needs, communication in MHPSS, and utilizing PSYCHLOPS. Over the project's duration, 6,694 physical rehabilitation sessions were provided to 2,043 beneficiaries, encompassing various age groups and genders. Each new beneficiary benefited from an average of 3-4 rehabilitation sessions. Aligned with beneficiaries' requirements, assistive devices were provided, with 2,291 devices distributed to 1,309 individuals in 2022. For individuals requiring amputation, prosthetics were seamlessly integrated into the multidisciplinary rehabilitation approach. In 2022, 280 prosthetics were supplied to 267 beneficiaries, contributing to their functional independence. Notably, 93% of individuals who completed the physical rehabilitation program reported an improvement in their functional autonomy. HI also delivered 3,445 education sessions for 979 caregivers, while 477 caregivers received a total of 1,577 psychological sessions. Furthermore, 25 awareness sessions reached 197 individuals, fostering understanding and inclusivity among both people with and without disabilities.
The project will contribute to improving the lives of persons with functional limitations in Northeastern Syria by enabling them to gain or re-gain their functional independence through rehabilitation services. The overall objective of the intervention is that Access to quality and GAD (Gender, Age and Disability) inclusive physical rehab services for vulnerable persons is improved, in three governorates in Northeastern Syria through a) Activities focusing on the strengthening of health facilities in providing quality services and b) Activities focusing on reinforcing beneficiaries awareness, participation and feedback on the services, in order to: 1) Increasing access: providing and scaling up physical rehabilitation services in existing rehabilitation units or supporting the setting up of services. Mobile teams will also do home visits to beneficiaries when they cannot come to the rehabilitation fixed point. 2) Strengthening the health system through close collaboration/capacity building of health facilities and training/coaching of staff members to provide a qualitative and patient-centred service. 3) Ensuring that beneficiaries are aware of the services and can access it, that they can provide feedback that will inform improvement and adaptation of the services. This overall objective will contribute to a longer term vision of health system strengthening where Syrias national health plan will, building on existing structures, ensure sustainable physical rehabilitation services at three different levels: Specialized rehabilitation services, intermediate rehabilitation services and community level rehabilitation services.
